Client Care Coordinator

Overview We are seeking a dedicated and organized Client Care Coordinator to join our team at Saline Heart Group. This position will provide initial patient contact, meet with eligible patients to present program, initiates enrollment into Intensive Cardiac Rehab, and schedules patient sessions. This position requires a strong understanding of medical administrative support, as well as familiarity with medical terminology. Duties: -Meet with eligible patients to present program, initiate enrollment, and schedule patient sessions. - Track eligible inpatients and follow up with potential program entrants upon discharge. - Ensure smooth transition as patients begin ICR program. - Coordinate with referring healthcare facility staff and monitor medical record data to capture eligible patients. - Coordinate with referring cardiologists for ICR-eligible patients. - Serve as the primary point of contact for clients, addressing inquiries, and providing information about services offered. - Schedule and confirm appointments for clients, ensuring optimal use of time and resources. - Maintain accurate client records, including personal information, appointment history, and billing insurance details. - Collaborate with medical staff to ensure seamless client care and communication. - Handle incoming calls and correspondence in a professional manner. - Support the overall efficiency of clinic operations by performing administrative tasks as needed. Requirements and/or Qualifications: - Strong organizational skills with attention to detail to manage multiple tasks efficiently. - Excellent verbal and written communication skills to interact effectively with clients and staff. - Proficiency in office software applications (e.g., Microsoft Office, Telemetry, Prognois, MedEolve) is necessary for record management. - Ability to work independently as well as part of a team in a fast-paced environment. - Extensive knowledge of the Pritikin ICR Program, CMS eligibility, and secondary insurance guidelines. - Formal education Presentation skills - Bachelor's Degree in related field. - Certification in CPR.
