Our mission statement definitely speaks to what La Clinica de Familia stands for, which is to empower and enrich families, individuals, and communities by providing quality medical, dental, behavioral health and educational service for people of all cultures.$25.25 to $33.94Non-ExemptJob SummaryThe CCBHC Patient Specialist provides a full range of professional nursing services, including assessment, planning, implementation, evaluation, and coordination of patient care in accordance with individualized, team-based Comprehensive Care Plans. Under the direct supervision of the CCBHC Clinical Team Lead, the CCBHC Patient Specialist plays a critical role in supporting individuals as they navigate behavioral health, physical health, and social service systems. The CCBHC Patient Specialist will work as part of a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery and wellness. This role addresses social determinants of health, identifies barriers and provides community-based resources and services.Core CompetenciesProvide patient-centered care - identify, respect and care about patients' differences, values, preferences, and expressed needs; relieve pain and suffering; coordinate continuous care; listen to, clearly inform, communicate with, and educate patients; share decision making and management; and continuously advocate disease prevention, wellness, and promotion of health lifestyles, including a focus on population health.
