Clinical Case Manager
Are you a self-motivated and highly passionate health care professional who enjoys working with senior-aged patients?
The Clinical Case Manager (CCM) for BRE provides administration and reporting for standardized behavioral health assessments in person to senior-aged patients, many of whom are in chronic pain or have chronic illnesses and depression. BRE is a rapidly growing company that provides ancillary mental and behavioral health services to medical practices. Through our proprietary software and dedicated leased staffing, BRE helps our clients deliver Medicare-supported services to provide more comprehensive care to their senior-aged patients.
Experience working with senior patients in a family practice medical office, nursing home or skilled nursing facility or similar setting is required. Candidates with a current CNA (Certified Nursing Assistant), CMA (Certified Medical Assistant), or LPN (Licensed Practical Nurse) is required.
This is an IN-OFFICE, FULL-TIME position, generally 30-40 hours per week working during regular daytime, clinic hours only (Typically 8am-5pm). NOTE: You must be available for 4-5 full clinic days (Monday-Friday). NO OVERNIGHT OR WEEKEND HOURS AVAILABLE.
Pay commensurate with experience.
Position Requirements:
· CNA (Certified Nursing Assistant), CMA (Certified Medical Assistant), or LPN (Licensed Practical Nurse) REQUIRED
· Experience working in a family practice medical office, nursing home or skilled nursing facility or similar setting is REQUIRED
· Work confidentially with the patients following HIPAA compliance standards, in a busy clinic setting
· The CCM needs to thrive in a highly independent work environment and be confident with using technology
· Excellent communication and interpersonal skills are a must
· Well-organized and compliant with completing required tasks
Responsibilities:
· Administration of assessments to patients in person
· Positive, professional and friendly demeanor with senior-aged patients
· Documenting daily patient notes (per patient) and creating patient activity calendars after each patient encounter
· Maintaining ongoing, open-lines of REMOTE communication with BRE's Clinical Services Manager and other BRE staff
· Proficient in Microsoft based software and various web-based software and technology

Pay: $24.00 - $26.00 per hour
