The PT will evaluate patients, develop treatment plans, and deliver therapy focused on improving mobility, strength, and overall functional independence.Must-Have Skills: Active Montana Physical Therapist License Strong patient assessment and treatment planning skills Experience in Skilled Nursing / Long-Term Care settings Knowledge of therapeutic exercises and mobility training Excellent communication and teamwork skillsExperience Required: Minimum 3 year of PT experience required SNF / Long-Term Care experience preferredKey Responsibilities:• Evaluate patients and develop individualized treatment plans• Provide therapeutic exercise, gait, balance, and mobility training• Monitor patient progress and adjust treatment plans as needed• Educate patients and caregivers on therapy techniques and safety• Maintain accurate and timely documentation• Collaborate with interdisciplinary rehab team• Promote patient independence and quality outcomesCloudinfraIT Inc Job ID #296029.
