Job TitleHospice Clinical Liaison RNLocation:Libertyville, IL (Advocate Condell in Libertyville )Job TypeFull-TimeSalaryUp to $92,000 per year (based on experience)OverviewJourneyCare Hospice is seeking a Registered Nurse (RN) to serve as a Hospice Clinical Liaison supporting hospital-based referrals at Lake Forest Hospital and Highland Park Hospital.This role is ideal for RNs with hospital or hospice experience who are interested in care transitions, patient advocacy, and working closely with interdisciplinary hospital teams. Hospice or liaison experience is helpful but not required—we provide training for strong acute care nurses transitioning into hospice.ScheduleMonday–Friday, 8:30 AM – 5:00 PMNo nightsHoliday rotation (1 summer, 1 winter)Local travel between assigned hospitalsResponsibilitiesPerform hospice eligibility assessments and bedside clinical evaluations in hospital settingsSupport hospice admissions and complete required documentation (CTI, consents)Collaborate with physicians, case managers, social workers, and discharge plannersAssist in determining hospice eligibility and appropriate level of careCoordinate transitions from hospital to hospice servicesEducate hospital staff and referral sources on hospice services and admission processBuild and maintain strong relationships with hospital partnersSupport urgent and General Inpatient (GIP) hospice referrals when neededQualificationsActive Registered Nurse (RN) license in IllinoisGraduate of an accredited nursing program1–2+ years of RN experience (hospital or hospice experience preferred)Strong communication and interpersonal skillsAbility to work in a fast-paced hospital environmentStrong clinical judgment and organizational skillsCurrent CPR certificationValid driver’s license and insurancePreferred ExperienceICU, med-surg, oncology, emergency department, or case management backgroundHospice or palliative care experience a plusExperience with care transitions or discharge planningExposure to patient education or family discussions in acute care settingsCompensation & BenefitsSalary up to $92,000 annuallyMedical, dental, vision, life, and disability insuranceBenefits effective first of the month after hire401(k) with company matchPaid time off and paid holidaysMileage reimbursementTuition reimbursementReferral bonus programCareer advancement opportunitiesAbout The RoleThis is a meaningful clinical position focused on supporting patients and families during important care transitions.
