Our mission is to enhance the quality of life for our patients through effective pain rehabilitation, medication management, and functional medicine strategies.Role SummaryWe are seeking a Nurse Practitioner or Physician Assistant to join our clinical team at From Pain to Wellness, LLC. In this role, you will be responsible for evaluating and managing patients with chronic pain conditions in collaboration with our physicians and interdisciplinary staff. You will play a central role in longitudinal patient care, contributing directly to treatment planning, medication management, and functional restoration.This position is designed for an APP who wants ownership of patient care within their scope, values interdisciplinary collaboration, and practices with a functional, whole-person approach to chronic pain.ResponsibilitiesConduct comprehensive patient assessments and physical examinations for chronic pain conditionsManage established chronic pain patients under physician supervisionDevelop and implement individualized treatment plans aligned with clinic protocols and care philosophyPrescribe medications and treatments in accordance with state regulations and clinical guidelinesMonitor patient progress and adjust treatment plans in collaboration with the care teamEducate patients on pain management strategies, expectations, and lifestyle considerationsCollaborate closely with physicians, therapists, and other clinical staff to ensure coordinated careFacilitate referrals to other healthcare professionals and diagnostic services as appropriateMaintain accurate, timely, and compliant clinical documentationQualificationsLicensed Nurse Practitioner or Physician Assistant in the state of Illinois, or eligible for licensurePrevious experience in chronic pain management or related specialty medical practice preferredExperience with medication management for complex patient populationsComfort managing a full patient panel within a structured clinical environmentStrong communication skills and ability to build rapport with patientsAbility to work collaboratively within an interdisciplinary care modelCommitment to following clinical protocols, documentation standards, and compliance requirementsExperience with or exposure to functional medicine principles aligned with chronic pain careWhy This RoleThis is a full-time role in a specialty medical practice with predictable hours, no on-call responsibilities, and a clearly defined care philosophy.
