In this role, you will provide comprehensive social work services to patients receiving home health care in Woodland Hills and surrounding areas. You will travel to patients’ homes to complete psychosocial assessments, identify barriers to care, and partner with patients, families, and caregivers to develop realistic, patient-centered care plans. You will serve as a key member of an interdisciplinary team, collaborating closely with nurses, therapists, and physicians to support safe, effective transitions of care and help patients remain in the comfort of their homes. Typical responsibilities include assessing emotional, psychosocial, financial, and environmental factors affecting patient health; linking patients and families to appropriate community resources; assisting with applications for benefits and entitlements; providing counseling and support during times of stress or crisis; and facilitating advance care planning, including discussions around goals of care and decision-making support. You may also help address issues such as caregiver burden, adjustment to illness, and coordination of in-home services or alternative levels of care when needed. A typical day involves scheduling and completing several home visits across the Woodland Hills and neighboring communities, documenting all encounters in the EMR in a timely manner, attending team case conferences, and participating in regular communication with clinicians to adjust care plans as patients’ needs evolve. Caseloads are designed to allow for meaningful interactions with each patient and family, focusing on high-quality, individualized care rather than rushed encounters. You will work with a supportive, collaborative team that values interdisciplinary input and respects the unique perspective social workers bring to patient care. The environment emphasizes coordinated, patient-centered care, evidence-based practices, and continuous professional growth. The home health setting allows for autonomy in the field while still providing structured support, mentorship, and access to resources from the broader organization.
