Updates Care Plans to reflect changes in client care and informs Management of same.Observant: Aware of a client's unique personal and medical history so as to recognize changes, whether behavioral, physical, social, or psychological as they pertain to a client's overall health and well-being.Clear & Concise: Communicates professionally, tactfully, and with discretion in verbal exchanges with the client, and family members.
