Blended Case Manager

PURPOSE: JFK's Blended Case Manager's provide community-based services in a variety of settings. BCM's may work from a participant's residence, rehab center, shelter, or correctional facility, etc. Environments for participant service delivery may include less than favorable conditions. BCM's assess needs and identify services & supports for clients coping with varying degrees of mental illness. BCMs: Ensure physical, emotional, social, intellectual and safety needs are met &maintained. Establish participant's short- & long-term goals for community living Link participants to chosen services and/or natural supports (i.e.- family, friends, neighbors etc.) Monitor the supports and services for appropriateness andeffectiveness. Advocate at all levels on behalf of participants We are seeking dedicated professionals who: Desire to embrace and advance the philosophy and objectives of JFK Behavioral Health Understand the social factors which contribute to attitudes and behaviors. Can foster effective relationships with Community Behavioral Health, Targeted Case Management, Family Court and other internal or external agencies to ensure quality service delivery. Are organized and can document daily service reports, strength & needs assessments, discharge/transfer summaries and daily clinical documentation to satisfy regulatory requirements. Are flexible, creative problem solvers that can adapt to evolving priorities. Are comfortable navigating the city by vehicle or public transit. Requirements: High school diploma plus a minimum of 12 college credit hours in sociology, social welfare, psychology, gerontology, or other social science plus two (2) years of mental health direct care experience. -OR- Certified Nurse Assistant (CNA) or Medical Assistant plus two (2) years of mental health direct care experience. Individuals using these criteria must receive a written waiver from the DBHIDS prior to hire.-OR- Bachelors degree in a Human Services discipline or other field that includes at least 12 credit hours in human services. Hours: 9am to 5pm;Work Week: The regular work week requires one weekend day butpreserves 1 full weekend a month off.
