Learn more about SWACH at www.southwestach.org The HealthConnect Specialist supports outreach, community engagement, and administrative efforts of the HealthConnect Hub across a wide range of projects and programs. This position works collaboratively across departments to support shared organizational goals and day-to-day operations. HealthConnect Hub operates as a regional community care hub that includes a cohort of community-based workers, a broad network of community and clinical partners, and a technology platform that supports whole-person care through coordinated program activities. The HealthConnect Specialist supports these efforts through coordination, training and technical assistance support, partner engagement, and care coordination operations that help the Hub function effectively. HealthConnect consists of a team. The Duties and Responsibilities listed below are encompassing of all of the assignments that could be allocated to a Specialist in the Department. Not every specialist in the HealthConnect Department will be responsible for the entire list, but should remain flexible to learning and developing within the department. Essential Duties and Responsibilities: Community Voice and Engagement · Support community engagement activities, including outreach events, tabling coordination, and participation in community meetings. · Assist with gathering and sharing community input to inform trainings, partnerships, and program improvements. · Support communication efforts that share information between HealthConnect, partners, and community stakeholders. Social Care Supports and Care Coordination Operations · Provide technical assistance and training to contracted partners and community-based workers to support care coordination and program activities. · Support referral coordination, ProviderOne verification, and documentation across care coordination workflows. · Carry out routine administrative tasks that support care coordination operations, including scheduling, tracking, and recordkeeping. Network Management and Capacity Building · Identify and engage community resource partners to strengthen the HealthConnect network. · Support partner relationships through meeting coordination, information sharing, and follow-up. · Assist with program evaluation, data tracking, reporting, and system support (e.g., CMS, Blackboard). Community-Based Workforce · Support development and delivery of community-based trainings and certifications. · Assist with learning management system (LMS) coordination and training logistics. · Support Community-Based Worker (CBW) activities through coordination, communication, and administrative assistance. · Connecting Community-Based Workers with state and local leaders and lawmakers for policy advocacy work · Representing the CHW workforce on local and statewide boards and committees · Facilitation of network meetings and trainings Administrative and Inclusivity · Advance equity in projects and across the organization · Prioritize work and manage time on an ongoing basis · Represent SWACH and all program partners with integrity and respect · Foster an inclusive culture of trust, respect, and mutual accountability · Role model of SWACH’s vision, mission, and values to all staff · Other duties as required Qualifications-Key Competencies and Traits · Demonstrated community outreach and mobilization skills in low-income, diverse, community settings. · Strong knowledge of community resources in SWACH region · Ability to support work as it evolves based on community and organizational needs. · Ability to work successfully independently and as a part of a team. · Demonstrated problem-solving skills. · Ability to establish and maintain effective working relationships with individuals from various groups and backgrounds, including team members and community members. · Understand and fully support the notion that people in marginalized communities are the experts about their own lives and have the best solutions to their own problems. · Experience with MS Office applications (Word, Excel, Outlook, PowerPoint, MS Project). · Proven ability to maintain high ethical standards and confidentiality. Qualifications-Education and Experience Any combination of education and experience that demonstrates the ability to perform the duties of the position is qualifying. This would typically include: · Bachelor’s degree in public health, community health, health education, public policy, social work or similar, or comparable experience. · Three to five years of experience working in social service or clinical settings, care coordination or community health work experience. · Experience understanding and addressing health equity and social determinants of health in diverse, underserved communities preferred. · Ability to pass background check and 10-panel drug screening Working Conditions & Physical Requirements Work is performed both virtually and in-person at SWACH office or at community sites as needed for team meetings, 1:1s, All-Staff meetings, trainings, and specifically assigned projects. Potential for travel among the three-county service area is required, including occasional evening and weekend meetings. · Ability to spend long periods working on a computer and using office equipment. · Using the telephone, virtual platforms, and/or face-to-face meetings to connect and collaborate with others. · Drafting, evaluating, editing, and reading documents.
