We offer flexible schedules, Travel Reimbursement, competitive health benefits, and continuing education benefits.Team 4: Amityville, Amityville Harbor, Amityville North, Copiague, Farmingdale, Lindenhurst, & Melville.3x 12 hour shifts - Thursday-SaturdayRotating WeekendsGreat Healthcare benefits, and education reimbursement of $5,250 annuallyAdditional compensation for visits over your caseloadJob DetailsOur Home Care RN's:Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.Measure vital signs, Administer IVs, change dressings, clean wounds as neededProvides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.Provide patient and family education for disease management and health promotion as directed by physician's plan of treatment.Provide care to the most upmost standardMinimum Qualifications:New York State RN licenseMinimum of one year of recent hands on experience as a Registered Nurse.Basic Life Support CertificationValid New York State Driver's License and reliable transportationSalary RangeUSD $100,000.00 - USD $130,000.00 /Yr.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
