The Physical Therapist will assess, plan, and implement individualized treatment programs to enhance patients’ mobility, manage pain, and improve their overall quality of life. Responsibilities include evaluating patients’ physical conditions, providing therapeutic treatments, collaborating with other healthcare professionals, and monitoring progress. Additionally, the Physical Therapist will ensure alignment with best practices and uphold the high standards of care set by We Care - Home Care and Hospice.QualificationsExpertise in Physical Therapy practices and Rehabilitation techniquesStrong foundation in Medicine and Patient Care, with a focus on safety and professionalismExperience providing Home Health Care and addressing individual patient needs in home settingsExcellent communication and interpersonal skills for patient engagement and collaboration with healthcare teamsAbility to work independently, manage schedules effectively, and deliver compassionate, client-centered careLicensed Physical Therapist in relevant state or eligible for licensureCPR certification or willingness to acquire one before startingPrevious experience in home care, hospice, or geriatric patient care is an advantage
