Generations is a non-profit, regional Federally Qualified Health Center (FQHC) that has proudly served eastern Connecticut since 1984. We provide comprehensive primary care, behavioral health, oral health, and case management services, primarily to medically underserved populations. We are currently seeking a Primary Care Registered Nurse (RN) to join our team at our Putnam location.

Location: Putnam, CT
Position Overview: As a Primary Care RN, you will provide high-quality, patient-centered nursing care within an ambulatory primary care setting. You will work closely with clinicians and clinical support staff to ensure safe, timely, and compassionate care while supporting chronic disease management, preventive services, and patient education.
Key Responsibilities
Perform nursing assessments and triage for scheduled, walk-in, and telephone encounters
Provide direct patient care through independent nursing visits, guided by clinician orders, standing orders, and established protocols
Provide ambulatory patient care, including medication and immunization administration, wound care, oxygen and emergency interventions, and point-of-care testing
Support chronic disease management and population health initiatives by educating patients and families, and promoting self-care and wellness
Coordinate patient care following emergency department visits, inpatient discharges, lab work, and medication changes; perform patient outreach to ensure timely follow-up and continuity of care
Collaborate with all members of clinical staff across departments to ensure smooth patient flow
Recognize changes in patient condition and respond appropriately in emergency situations
Participate in Quality Assurance and Performance Improvement initiatives
Assist with regulatory compliance, policy development, and grant-related activities as appropriate
Support community-based services and public health initiatives as needed
Schedule & Compensation:
Hourly Rate: $35.00+ per hour (commensurate with experience)
Schedule:
Monday-Friday, 8-hour shifts
Occasional evenings and one Saturday per month may be required based on operational needs
