Behind every placement is our experienced operations team handling compliance, credentialing, and communication so you can focus on delivering exceptional care.Catalytic Solutions—where clinicians thrive, partners trust us, and healthcare staffing gets smarter.**Empower Lives as a Master of Social Work!** \n\n**Job Description:** \n- Join a dynamic team providing crucial social services and support. \n- Work a 5x8-hour day shift from 08:00 to 16:30. \n- Collaborate in an outpatient clinic and private practice settings. \n- Conduct Biopsychosocial assessments to evaluate client needs. \n- Assist patients in navigating the Department of Children and Families (DCF) process. \n- Coordinate care and services for patients transitioning from hospital to home. \n- Document essential information to ensure smooth care coordination. \n- Participate in care conference activities to discuss patient progress. \n- Refer clients to rehab and long-term care facilities when necessary. \n- Engage in individual, marital, and parental counseling sessions. \n- Leverage knowledge of community programs including AFDC, Food Stamps, Medicaid, Medicare, and TANF. \n- Provide guidance on protective services, including adult and child protective services, emergency detention, guardianship, and power of attorney. \n- Utilize specialized knowledge in adoption/foster care and drug/alcohol treatment options. \n- Participate in the development of treatment plans for clients in psychiatric hospital settings, including 5150/involuntary holds. \n\n**Why Catalytic Solutions (CatSol):** \nAt CatSol, we connect dedicated therapists with rewarding assignments across the country.
