Home Health Physical Therapist (CONTRACT)
Position: Home Health Physical Therapist
Pay: $60-70/Hour PLUS MILEAGE
Key Responsibilities:
Patient Evaluation
Conduct comprehensive assessments of patients’ physical abilities, strengths, and functional limitations.
Treatment Planning
Develop individualized treatment plans based on evaluations, medical history, and physician orders.
Therapy Implementation
Provide skilled physical therapy interventions including therapeutic exercise, manual therapy, and modalities to improve strength, balance, mobility, and overall function.
Patient & Family Education
Educate patients and caregivers on exercises, body mechanics, and strategies to enhance safety and independence.
Progress Monitoring
Monitor patient response to treatment and adjust plans of care as clinically appropriate.
Collaboration
Communicate and collaborate with physicians, nurses, and other members of the interdisciplinary care team.
Documentation & Compliance
Complete accurate, timely documentation in accordance with agency, state, and federal guidelines.
Ensure all services meet legal, ethical, and regulatory standards.
Discharge Planning
Participate in discharge planning, including functional reassessment and recommendations for continued care or community resources.
QualificationsEducation
Doctor of Physical Therapy (DPT) from an accredited program.
Licensure
Active, unrestricted Physical Therapist license in Washington State.
Experience
1–2 years of experience as a Physical Therapist.
Home Health or Skilled Nursing Facility experience required.
Home Care Home Base experience preferred
Skills & Competencies
Strong clinical knowledge and evidence-based practice skills.
Excellent communication and interpersonal abilities.
Ability to work independently and manage a home-based caseload.
Strong organizational skills and attention to detail.
Compassionate, patient-centered approach to care.
Certifications
Current CPR/BLS certification.
Experience or certification in geriatrics and/or care of vulnerable populations preferred.
Physical Requirements
Ability to lift up to 50 pounds and assist with patient transfers.
Ability to bend, kneel, and perform physical tasks throughout the workday.
Ability to travel to patient homes within the designated service area.
