Physician Internal Medicine-Geriatrics - Competitive Salary

Job Description BC/ Internal Medicine or Family Medicine or Geriatric Medicine Seeking a Pod Leader for Virginia Beach, VA This is a full-time physician role caring for Medicare and Medicaid geriatric patients in the home setting. The Pod Leader will spend 60% of their time seeing their own patient panel in homes or assisted living facilities. 40% of the time will be in the office conducting meetings and supporting the other providers in the pod. Generally, there will be 2-4 providers in each Pod (not including leader) The Physician Team Leader (PodLeader) acts as a resource/coach impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team. Experience with HEDIS and quality metrics required Success in Value Based Care changes the demands placed on primary care.
