Behavioral Health Case Coordinator
Reporting to the Behavioral Health Program Manager, this role will be responsible for the full case management of the assigned caseload and coordination of appropriate services for clients. This position provides individualized teaching, training, and support needed to adults with severe and persistent mental illnesses to help them maintain their independence in the community. Case Coordinators will work with clients in their homes and throughout the community to provide life skills training and case management services.
This role assumes responsibility for advocating for, coordinating the delivery of, and monitoring a wide variety of services and supports for eligible persons to enable them to lead meaningful and productive lives.

Bachelor’s degree in Human Services or related field with two years of relevant experience.
High School Diploma/GED and five years’ experience can be substituted for a degree.
Valid driver’s license required and access to reliable transportation.
At least 2 years of driving experience required.
Demonstrated ability to work effectively in a diverse work environment.
Experience in Behavioral Health environment a plus.
