Job Description & Requirements Specialty: Home Health Discipline: RN Start Date: 05/25/2026 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Position Overview We are seeking an experienced Registered Nurse Case Manager to provide skilled, patient-focused home health care within Luzerne County. This role is responsible for managing a designated territory and delivering comprehensive nursing services across the full continuum of home health care. Each RN Case Manager maintains primary ownership of their caseload while supporting team coverage as needed to ensure continuity of care. Schedule & Work Details Schedule: Monday through Friday, 8:00 AM to 4:30 PM Weekly Hour Guarantee: 32 hours Productivity Expectation: 30 points per week Coverage Area: Luzerne County On-Call/Weekend Requirement: Every third weekend and one night of on-call coverage per week Mileage Reimbursement: Yes, reimbursed between patient visits Pay Rate: $51 to $53 per hour Key Responsibilities Serve as RN Case Manager for an assigned territory, providing 100 percent direct patient care Perform skilled nursing visits including routine visits, start of care, resumption of care, recertifications, and discharges Complete comprehensive patient assessments and manage individualized plans of care Coordinate care with physicians and interdisciplinary team members to ensure high-quality outcomes Maintain accurate, timely documentation using the electronic medical record system Support team coverage by assisting with patient visits during scheduled absences to ensure continuity of care Adhere to agency policies, infection control standards, and state, federal, and CMS regulations Required Qualifications Active Registered Nurse license Ability to work independently in a field-based role Strong clinical assessment, organization, and documentation skills Preferred Qualifications Recent home health nursing experience Experience using Homecare HomeBase (HCHB) Excellent time management and care coordination skills About PRIDE Health PRIDE Health is the minority-owned healthcare recruitment division of Pride Global—an integrated human capital solutions and advisory firm.
