Primary clinical responsibilities will include the evaluation, treatment, and management of patients. Requesting care for acute issues which may include also screening for all chronic diseases.Key ResponsibilitiesInitiates an appropriate assessment of person health status, ranging from focused to comprehensive Identifying immediate needs which can be supported or the need for a referral for long term managementPerforms physical examinations, performing and documenting a health history as evidenced by chart review and ability for others to understand patient needsConsults with colleagues, specialists, and other health professionals as appropriateOrders pertinent diagnostic tests and procedures to support recommended therapySupports obtaining or performs specimen collection for commonly performed blood, urine, and stool analyses and culturesInterprets health information and establishes diagnoses consistent with the accumulated data and formulates treatment plan, ensuring timely provision of necessary health care servicesProvides education and ensures understanding of members regarding the nature of the identified acute needs and further opportunities for evaluation and treatmentParticipates in the supervision of mid-level providers, including Nurse Practitioners/Physician AssistantsEducates patients regarding health promotion and disease preventionPromotes effective working relations and works effectively as part of a team to facilitate the ability to meet goals and objectives, around timely, efficient care and highly reliable customer serviceAttends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, specific job–related hazardsMaintains work area and equipment in condition required by Legacy standardsFollows Legacy’s exposure control plans/blood-borne and airborne pathogensParticipates in routine environmental audits and communicates needs as appropriateDemonstrates knowledge of techniques, procedures, and correct use of protective barrier equipment (Universal Precautions)Assures a safe environment by instituting appropriate control measuresAttends and completes annual required education programsPerform other duties as assignedMinimum QualificationsMD/DO: Medical Doctor with current licensureMD/DO: Board certification (or Board eligible) requiredProvider shall obtain, maintain, and participate in the Medicare and Medicaid Programs, workers ‘compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by LegacyAbility to meet Legacy Community Health’s EPIC proficiency requirementsPrevious experience in a community healthcare setting is preferredDemonstrated competency with an EHRS systemExcellent communication skills and customer service skillsAbility to multitask in a fast-paced environmentAbove average organizational skillsStrong typing and computer skillsThis job requires access to confidential and sensitive information, requiring ongoing discretion and secure information managementMust be able to manage multiple deadlines and decision-making responsibilitiesAbout Legacy Community HealthAs the largest Federally Qualified Health Center (FQHC) in Southeast Texas, Legacy Community Health has been delivering comprehensive, high-quality, and affordable health care for nearly 40 years.
