Remote Clinical Pharmacist - Contract
Job descriptionWe are seeking a licensed pharmacist to join our clinical pharmacy team to support and improve healthcare outcomes related to Statin Use in Persons with Diabetes (SUPD) and Polypharmacy-Anticholinergics (Poly) measures. This role will involve performing direct telephonic outreaches to providers, pharmacies, and members to address gaps in care, improve medication management, and optimize patient health outcomes.Pay Rate (Hourly Wage):$60.00Location:100% RemoteExpected Hours: 40 hours per weekContract: 7-month contractJob Type: ContractRESPONSIBILITIES/TASKS:Conduct telephonic outreach to healthcare providers, pharmacies, and members regarding Statin Use in Persons with Diabetes (SUPD) and Polypharmacy-Anticholinergics (Poly) measures.Educate providers and pharmacists on current guidelines for statin therapy in diabetes and managing polypharmacy, especially anticholinergic medications.Identify and close care gaps related to statin use and polypharmacy by reviewing patient data and collaborating with providers and pharmacies.Provide members with personalized counseling and education on managing medications.Track and report progress on quality measure performance and provide actionable insights to improve ratings.Collaborate with clinical teams, to ensure comprehensive care for patients.Educate members on medication management and encourage adherence to prescribed statin therapy and polypharmacy risk reduction.Stay updated on clinical guidelines and treatment protocols.Participate in program development and continuous improvement efforts to optimize outreach strategies and enhance care delivery.Perform other clinical duties as required.This position description outlines typical responsibilities and tasks but may require other essential functions.EDUCATION OR EQUIVALENT EXPERIENCE:Bachelor's Degree required.Doctor of Pharmacy (PharmD) degree required.Active pharmacist license in good standing.EXPERIENCE:At least 3-5 years of experience in retail, clinical, or managed care pharmacy.Experience in Statin medications and prescription benefits.Call center management is preferred but not required.Knowledge of prescription medications and therapeutic classes.Experience in program management and collaboration with healthcare teams.Engage directly with provider offices and conduct telephonic outreach regarding clinical recommendations.Handle provider pushback professionally and persuasively when discussing recommendations.Demonstrate background beyond retail pharmacy – such as hospital, managed care, or specialty pharmacy.Show confidence and strong communication skills, especially in situations that may be uncomfortable in pharmacy but help drive outcomes.Balance clinical impact and metrics.SKILLS/KNOWLEDGE/ABILITIES (SKA) REQUIRED: (Brief bullet points detailing the skills necessary for this position)Strong clinical knowledge and experience with a wide range of medications.Excellent verbal and written communication skills.Strong project management skills.High proficiency in Microsoft Office and healthcare software.Ability to work independently and in a team environment.Strong customer service and interpersonal skills.Ability to work effectively in a remote environment.WORKING CONDITIONS:Work is performed in a remote setting with no unusual hazards.The qualifications listed above are intended to represent the minimum education, experience, skills, knowledge and ability levels associated with performing the duties and responsibilities contained in this job description.We are an Equal Opportunity Employer.
