Role OverviewAvail Health is building a behavioral health and dementia care program supporting Medicare patients with serious mental illness, complex psychiatric conditions, substance use disorders, and dementia-related needs. The Psychiatric Mental Health Nurse Practitioner (PMHNP) is the clinical psychiatric anchor of that program - delivering comprehensive psychiatric evaluation, medication management, and dementia care via telehealth within a highly coordinated interdisciplinary team.In addition to direct clinical care, the PMHNP serves as the GUIDE Provider of Record for Avail Health's participation in the CMS GUIDE (Guiding an Improved Dementia Experience) model, providing clinical oversight and serving as the authoritative clinical voice within the program's dementia care framework.You'll work closely with the LCSW, Dementia Care Navigator, and Care Coordinator, supported by an ambient AI scribe and technology-enabled workflows designed to reduce administrative burden and support high-quality virtual care delivery.This is a foundational clinical role in a newly launching program.What You'll OwnPsychiatric evaluation, medication management, and clinical decision-making for a complex adult and geriatric Medicare patient panelClinical oversight of the CMS GUIDE dementia care program as Provider of RecordEvidence-based SUD diagnosis and medication-assisted treatment in coordination with the LCSWWhat You'll DoConduct comprehensive psychiatric diagnostic evaluations including history-taking, mental status examination, and DSM-5 diagnostic formulation across mood, anxiety, psychotic, trauma-related, and late-life psychiatric presentationsPrescribe, titrate, and monitor psychotropic medications with attention to geriatric pharmacology, polypharmacy risks, drug-drug interactions, and comorbid medical conditionsAssess, diagnose, and provide pharmacological treatment for substance use disorders including initiation, titration, and monitoring of medication-assisted treatment with buprenorphine/naloxoneEvaluate and manage patients across the dementia spectrum, including pharmacological management of behavioral and psychological symptoms of dementia (BPSD)Serve as GUIDE Provider of Record - providing clinical oversight for care plans, partnering with the Dementia Care Navigator and LCSW, and contributing to dementia care protocol development in alignment with CMS requirementsSupport patients transitioning from inpatient psychiatric or medical settings, including medication reconciliation, psychiatric stabilization, and care continuityConduct clinical risk assessments, develop individualized safety plans, and coordinate escalation with care team members and emergency services when indicatedParticipate in interdisciplinary huddles and case review with the LCSW, Dementia Care Navigator, Care Coordinator, and clinical leadershipComplete all clinical documentation accurately within 48 hours of encounter, meeting applicable regulatory, clinical, and billing standardsContribute frontline clinical insight to support workflow refinement, protocol development, and scalable program infrastructure as the market growsWhat Success Looks LikeHigh-quality psychiatric evaluation and medication management delivered consistently, with clear clinical reasoning and measurable patient outcomesBPSD effectively managed - reducing behavioral crises, optimizing medication titration, and improving quality of life for patients and caregiversGUIDE program clinical oversight reliable and CMS-compliant, with actionable guidance to navigation staff and strong program quality outcomesPost-hospitalization transitions supported seamlessly, with psychiatric stabilization and community reintegration on trackDocumentation completed accurately and on time, meeting all compliance and billing requirementsWhat You BringRequired:MSN or DNP with a specialization in psychiatric mental health nursing from an accredited programActive, unrestricted APRN licensure in the State of Maryland in good standingBoard certification as a PMHNP-BC through ANCC or equivalent certifying bodyActive Maryland DEA registration including Schedule III-V controlled substance prescribing authorityMinimum 5 years of post-certification clinical experience in psychiatric advanced practice with demonstrated expertise in psychiatric evaluation, medication management, and geriatric mental healthExtensive clinical experience in dementia evaluation and management, including pharmacological management of BPSDClinical experience in SUD diagnosis and treatment, including initiation and monitoring of medication-assisted treatmentExperience ordering and coordinating home health services including behavioral health nursing, PT, and skilled nursingProficiency with EHRs and telehealth platforms; comfort with technology-enabled clinical workflowsReliable high-speed internet and a dedicated, HIPAA-compliant home workspacePreferred:Fluency or conversational proficiency in SpanishFellowship or advanced training in geriatric psychiatryExperience as a provider of record within a CMS Innovation Center model or similar value-based care programFamiliarity with the CMS GUIDE model or other dementia-focused care navigation programsFamiliarity with Medicare Advantage, TCM, or value-based care modelsExperience with ambient AI scribe tools or other technology-enabled clinical platformsExperience within startup healthcare organizations, new program launches, or rapidly scaling care environmentsSchedule and Work StyleWork Type: Fully remote - virtual telehealth deliverySchedule: Monday-Friday, 8:00 AM - 5:00 PM ETTravel: Preference given to candidates residing in Maryland or within commuting distance for occasional in-person meetings or program activitiesAutonomy: High - serves as the psychiatric clinical anchor for a complex interdisciplinary care teamCompensation and PerksThis role is structured as a part-time hourly position during the initial program launch phase, with a target transition to full-time salaried status as patient panel volume grows. The anticipated full-time annualized salary range is $150,000-$170,000 ($72-$82/hour), commensurate with experience.
