You'll provide customized care plans and therapeutic interventions, helping individuals regain independence and enhance their quality of lifeall from the comfort of their homes. Key Responsibilities:Evaluate and assess patients' physical conditions to develop tailored treatment plansImplement and adjust physical therapy interventions focused on mobility, strength, balance, and functionEducate patients and caregivers on therapeutic exercises, techniques, and safety precautionsCollaborate with physicians and other healthcare professionals to coordinate patient careMaintain accurate, timely documentation of patient progress and treatment plansParticipate in on-call rotation (if required) Qualifications:Licensed Physical Therapist (PT) in CAMinimum of 1 year of clinical experience xmcpwfu (home health preferred but not required)Excellent communication, problem-solving, and organizational skillsAbility to work independently and manage a flexible scheduleReliable transportation and valid drivers licenseKnowledge of Medicare/Medicaid guidelines is a plusEMR experience is preferredFlexible work from home options available.
