Speech Language Pathologist

Description:The speech language pathologist is responsible for providing a full range of speech-language pathology services including identification, assessment, treatment planning and therapeutic interventions in an interdisciplinary environment.Essential Functions:1. Conducts appropriate evaluations, assesses home environment (as appropriate), and identifies equipment needs relative to speech/language function.2. Identifies functional speech defects and establishes a Plan of Care (POC) to improve patient's function.3. Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.4. Recommends communication devices/aids as indicated.5. Confers with referring physician and other staff to ensure coordinated and comprehensive care contributes to the total Plan of Care (POC).6. Documents the course of patient care including progress made and continuing need for treatment.7. Provide patient and family/caregiver education on tasks and resources that will assist with continued functional independence.8. Identifies patient and family/caregiver needs for other services and refers as appropriate.9. May supervise home health aide in procedures directly related to the speech-language therapy Plan of Care (POC).10. As appropriate, participates in case conferences, in-services and other meetings required to ensure coordinated and comprehensive care.11. Performs other related duties as assigned or requested.Knowledge/Skills/Abilities:1. Demonstrates good verbal and written communication and organization skills.2. Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others.3. Meets mandatory continuing education requirements. Maintains clinical competency in speech-language pathology practice and theory.4. Demonstrates commitment, professional growth and competency.5. Ability to work flexible schedule and travel locally.6. Ability to do heavy lifting, bending, pulling, pushing and standing. Prolonged standing and walking required. Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psychosocial needs.7. Requires hand-eye coordination and manual dexterity.8. Promotes company philosophy and administrative policies.9. Must read, write and speak fluent English.SkillsMust possess current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from the American Speech-Language-Hearing Association (ASHA) or be CCC eligible and hold current state license as applicable - RequiredProof of current CPR certification - RequiredCurrent driver's license - RequiredMust have reliable transportation and insurance - Required1 year clinical experience in Speech-Language Pathology - RequiredExperience in a home care environment - PreferredEducationGraduate of a master's level program in speech-language pathology which is either accredited by the Educational Standards Board or whose course work and practice meet ASHA certification requirements - RequiredQualificationMust HaveHealthcare
