Registered Nurse - Community Care Coordinator

The Community Care Coordinator Registered Nurse is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. The incumbent assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.Duties will include but are not limited to:Ensure patients receive coordinated care services across the continuum of care; working collaboratively with PACT, specialty and outpatient teams to coordinate optimal care services.Plans, evaluates, and delivers services by working collaboratively with care team, business office, and other internal and external service departments.Assists in the coordination of Care in the Community ( CITC ) between Veteran Affairs ( VA), outside facilities, and physician offices.Gathers patient appointment information and medical documentation of care provided.Provides medical documentation of care to HIMS staff for scanning into patient's medical record.Assist in reviewing and approving consults based on established criteria and service agreements.Reviews consult for approval, appointment verification, and obtaining appropriate documentation.Reviews Secondary Authorization Requests from Community Providers for appropriateness and arranges services when necessary.VA offers a comprehensive total rewards package: VA Nurse Total RewardsPay: Competitive salary, regular salary increases, potential for performance awardsPaid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VAInsurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)Licensure: 1 full and unrestricted license from any US State or territoryWork Schedule: 7:30am - 4:00pm, Monday - FridayTelework: Ad-HocPermanent Change of Station (PCS): Not Authorized
