Community Health Worker (Immokalee Training Site - College of Medicine)

DepartmentImmokalee Training Site - College of MedicineResponsibilitiesProvide community-based support, outreach, and care navigation for children, families, and pregnant or parenting individuals with mental health needs. Coordinate patient support services, including screenings, psychoeducation, and linkage to care.Deliver training and support to Community Health Workers and contribute to workforce development initiatives.Engage in community outreach to promote early identification of concerns, reduce stigma, and connect individuals to health and social services.Collaborate with interdisciplinary teams and support research and program activities.Contribute to the education of healthcare trainees through community-informed perspectives and experiential learning opportunities.QualificationsBachelor’s degree in social work, public health, psychology, or a related field and two (2) years of relevant experience in community-based services, case management, behavioral health, or social services. (Note: higher education may substitute for experience at the equivalent rate.)Valid FL driver’s license or ability to obtain prior to hire. Ability to travel locally.Preferred QualificationsExperience working with children, families, or pregnant and parenting individuals.Knowledge of community health systems, mental health services, and care navigation.Experience in community outreach, health education, or Community Health Worker initiatives.Bilingual (e.g., English/Spanish) strongly preferred.Familiarity with trauma-informed care and responsive practices based on individual needs.Contact InfoJavier I.
