Position : Case Manager (Outreach)Location : Olympic, Downtown Los Angeles, CADepartment : E6Schedule : Monday-Friday 8:00am-4:30pmStatus : Full time, Non-exempt, Non-managementSummaryWith the support of and in partnership with the E6 team, the Case Manager performs a variety of tasks to assist the most vulnerable and chronically homeless individuals, and is responsible for identifying, engaging and providing support to homeless clients, including aftercare services and follow up support, once housed. The Case Manager will focus specifically in the Midtown Los Angeles area and will engage homeless individuals, as well as provide linkages to housing and supportive services. These teams are part of a large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services. The Case Manager provides information and support for program members, and assists members in linking to a variety of resources and services as needed. The Case Manager will model competence in wellness, recovery, and coping, and is responsible for maintaining the highest quality services and efficiency of operations.Essential Duties and ResponsibilitiesIdentify, engage, and provide support to clients in the E6 program living on the streets in the Midtown Los Angeles area, targeting those clients who are vulnerable due to medical, psychiatric, or addiction needs. Work in collaboration with other team members to remove barriers to permanent housing; e.g., acquisition of identification, legal aid, benefits assistance, life skills acquisitionWork collaboratively as a team to identify appropriate services, taking into account each individual's physical and cognitive needsProvide case management and aftercare/follow-up services to clients placed in permanent housingReport all statistics and updates for reporting purposesProvide initial contact, needs assessment, information, education and referrals to the homelessProvide case management, basic services, and advocacy for clientsRefer target clientele to appropriate services (e.g., health, mental health, interim housing, etc.)Develop effective, trusting relationships with clients that facilitate provision of servicesWork in collaboration with E6 team members and outside agency staff to connect clients to appropriate servicesMaintain and record accurate electronic records, client files, and statistical informationAttend all case file, staff meetings, trainings and retreatsParticipate in CES outreach meetings and other community meetings as assigned.Will need to transport clients in personal vehicle at times, thus valid CA license and personal insurance is required.QualificationsBA degree or equivalent experienceDemonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)Willingness to work in a high tolerance Harm Reduction model with clients who have multiple barriersWorking knowledge of social services agencies in Metro LA (SPA 4) and LA County agencies serving the homelessExcellent human relations/communication skills; ability to act in professional manner regardless of circumstancesSkilled in non-violent crisis interventionAbility to self-motivate and organize time effectively and efficientlyComputer literate; able to use computerized database system for information managementPassion for working with the population servedAbility to work as part of an interdisciplinary teamExpected Behaviors of All StaffAct as a role modelDemonstrate a sense of responsibilityContinuously learn and improveAcknowledge your own areas of improvementHear and provide honest, specific and direct feedbackCreate an environment where everyone is welcomed, valued and respectedCollaborateEqual Opportunity EmployerThe People Concern is an equal opportunity employer dedicated to non-discrimination in employment.
