Responsibilities:
The primary responsibility of the Social Services Director in the operation of the Social Services Department in accordance with current federal, state and local standards to ensure that the social needs of the residents are met.
Oversight of the facility’s Social Services Department and ensuring that the needs of the residents are met on an individual basis
Interviewing resident/families to obtain social history and involving the resident/family in planning objectives, goals and social services programs
Assisting in the development of a written plan of care for each resident that identifies the problems/needs, the goals to be accomplished, and the recommended approach for each
Interpreting social, psychological and emotional needs of the resident to the medical staff, attending physician and other resident care team members
Arranging psychological care and evaluation services as needed
Assisting in the scheduling of care plans and assessments to be presented and discussed at each committee meeting as needed
Ensuring that all social services personnel are aware of the care plan
Reviewing nurses notes to determine if the care plan is being followed
Chairing the Behavior Management and Antipsychotic Committee

Qualifications:
Social Services Director Qualifications
Must possess a Bachelor’s Degree from an approved school of Social Work (BSW, MSW, CSW’s are preferred) or a Bachelor’s in a human services field including but not limited to sociology, special education, rehabilitation counseling and psychology
A minimum of one year of supervised social work experience in a health care setting working directly with patients
A Social Services Designee form the state of employment if the individual does not possess a Bachelor’s Degree
Have a thorough understanding of the principles of best Social Services practices
