Center Medical Director

Position OverviewThe Center Medical Director (CMD) leads the clinical team at Oak Street Health, providing primary care (80% clinical, 20% administrative) and ensuring high‑quality patient outcomes across the center.Key ResponsibilitiesSupport organizational strategy and drive implementation of OSH strategic initiatives.Collaborate with regional leadership to execute regional priorities and reach objectives.Coordinate and lead onboarding of new providers, model excellent medical care, and support care model implementation.Ensure evidence‑based care, teach clinical guidelines, and provide preventive care initiatives.Educate providers on value‑based full‑risk model and address metric goals.Create a culture of coaching, conduct performance reviews, and address provider and team feedback.Celebrate successes, support a positive Oaky culture, and achieve triad best practices.Perform other duties as assigned.QualificationsM.D., D.O., A.P.N., or P.A. with requisite board certification (Internal Medicine or Family Medicine for M.D./D.O., or Family, Adult, or Gerontology for A.P.N.).5+ years of experience in outpatient practice (preferred); fellowship training in Geriatrics or other professional degrees welcomed.Prior clinical leadership experience, coaching providers and motivating teams.Experience using a metrics‑driven approach to medical care and quality projects.Strong leadership skills, ability to handle ambiguity, and passion for innovation.Commitment to older adults and low‑income patients.US work authorization.Additional RequirementsAnticipated Weekly Hours: 40. Time Type: Full time.
