Job Description for Physical TherapistJob Title: Physical TherapistLocation: Portland, ME 04101Guaranteed Hours: 40 hours per weekShift: Monday to Friday, until shift is complete. On-call required if requested.Travel Area: Cumberland County, covering towns including Pownal, Freeport, Durham, Brunswick, Bath, Topsham, Bowdoin, Bowdoinham, Lichfield, Richmond, West Gardiner, and Gardiner.Productivity Units: 30 unitsPay package:Total: $2560Weekly/40 HoursLodging + Meals (Non - Taxable) - $1920/WeekW2 (Taxable)- $16/Hr. ($640 Weekly)Description: A registered professional who provides physical therapy to patients of all age groups. The Physical Therapist plans, implements, and evaluates patient care plans to restore or maintain patient well-being. They plan, organize, and conduct physical therapy treatment programs based on medical referrals and evaluations, collaborating with an interdisciplinary team.Essential Functions:Perform clinical assessments to identify patient needs and appropriateness of care.Document, prepare, review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical records on an ongoing basis. Revise the POC based on evaluation and ongoing assessment data, working in consultation with the physician and as part of an interdisciplinary team.Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referrals and therapy assessments.Assist/instruct patient/family in transfer techniques appropriate to patient abilities. Instruct on the use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs. Order appropriate rehabilitative equipment to meet patients' needs and adapt equipment/appliances to meet home limitations/requirements.Provide appropriate patient/family education regarding therapeutic procedures based on the patient's age, condition, culture, and learning readiness.Perform and oversee other team members, when applicable, in the performance of patient care consistent with patient needs and desires.Apply concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and disease transmission.Accept clinical assignments consistent with education and competence to care for patients.Perform other related duties as assigned or requested.Knowledge/Skills/Abilities:Effective interpersonal relations and communication skills.Knowledge of durable medical equipment.Meets mandatory continuing education requirements.Demonstrates commitment, professional growth, and competency.Ability to work a flexible schedule and travel locally.Requires hand-eye coordination and manual dexterity.Promotes company philosophy and administrative policies.Must read, write, and speak fluent English.Skills:Current State License as a Physical Therapist - RequiredProof of current CPR certification - RequiredCurrent driver's license, reliable transportation, and insurance - Required1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - RequiredPrevious home care experience - PreferredEducation:Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - RequiredQualifications:Recent home health experience preferredExperience with Home Care Home Base EMR preferred
