# CARE BASE LLC ## JOB DESCRIPTION – PROGRAM MANAGER \*\*Position:\*\* Program Manager \*\*Company:\*\* Care Base LLC \*\*Employment Type:\*\* Full-Time \*\*Annual Salary:\*\* \*\*$50,000 per year\*\* \*\*Work Setting:\*\* Residential Group Home / HCBS Services \*\*Reports To:\*\* Director of Operations / Company Management ### POSITION SUMMARY Care Base LLC is seeking a \*\*qualified and experienced Program Manager\*\* to oversee the daily operations of a residential group home serving individuals with \*\*intellectual and developmental disabilities (I/DD)\*\*. The Program Manager will be responsible for ensuring that individuals receive safe, respectful, person-centered, and high-quality services in accordance with their \*\*HCBS waiver services, individual service plans, applicable state and federal requirements, and Care Base LLC policies and procedures.\*\* The Program Manager will provide leadership to direct-support professionals, coordinate services, monitor documentation, manage staffing, address health and safety concerns, and ensure the group home operates efficiently and remains compliant with all applicable requirements. ## KEY RESPONSIBILITIES ### 1. Group Home Management \* Oversee the day-to-day operations of the Care Base LLC group home. \* Ensure the home remains safe, clean, organized, and comfortable for individuals served. \* Ensure individuals receive services according to their approved service plans and identified needs. \* Coordinate daily activities, appointments, transportation, meals, personal care, and community activities. \* Promote independence, dignity, choice, community participation, and quality of life. ### 2. HCBS WAIVER SERVICES & COMPLIANCE \* Demonstrate working knowledge and experience with \*\*Home and Community-Based Services (HCBS) waiver programs\*\*. \* Ensure services are delivered in accordance with applicable HCBS requirements. \* Monitor compliance with individual service plans and authorized services. \* Ensure required documentation is accurate, complete, timely, and maintained appropriately. \* Monitor incident reporting, medication documentation, progress notes, service records, and other required documentation. \* Assist Care Base LLC in preparing for audits, inspections, licensing reviews, and quality-assurance activities. \* Identify compliance concerns and implement corrective actions when necessary. ### 3. STAFF SUPERVISION \* Supervise and provide leadership to direct-support professionals and other residential staff. \* Develop staff schedules and ensure appropriate staffing coverage. \* Train, coach, and mentor employees to provide quality services. \* Monitor staff performance, attendance, professionalism, and compliance with company policies. \* Conduct staff meetings and provide ongoing guidance and training. \* Address employee performance issues appropriately and communicate significant concerns to management. \* Ensure staff maintain required training, certifications, and qualifications. ### 4. INDIVIDUAL SUPPORT & CARE COORDINATION \* Ensure individuals are treated with dignity, respect, compassion, and fairness. \* Monitor individuals' physical, emotional, behavioral, and social well-being. \* Coordinate medical, dental, therapy, psychiatric, and other appointments as needed. \* Communicate appropriately with guardians, families, case managers, healthcare providers, and other members of the individual's support team. \* Participate in service planning meetings and advocate for the needs and rights of individuals. \* Report significant changes in an individual's condition or needs promptly. ### 5. HEALTH, SAFETY & MEDICATION OVERSIGHT \* Maintain a safe residential environment for individuals and staff. \* Ensure staff follow approved medication administration procedures. \* Monitor medication records and promptly address medication errors or concerns. \* Ensure emergency procedures and safety plans are maintained and followed. \* Ensure incidents, accidents, injuries, suspected abuse, neglect, exploitation, or other serious concerns are reported according to applicable requirements. \* Respond appropriately to emergencies and urgent situations. ### 6. DOCUMENTATION & RECORDS \* Review daily staff notes and service documentation for accuracy and completeness. \* Ensure documentation supports services actually provided to individuals. \* Review medication administration records and other required records. \* Ensure incident reports are completed and submitted within required timelines. \* Maintain confidentiality of individual and employee information. \* Correct documentation deficiencies and provide staff with appropriate direction. ### 7. QUALITY ASSURANCE \* Regularly monitor the quality of services provided at the group home. \* Conduct routine reviews of staff performance, documentation, health and safety, and residential conditions. \* Identify areas for improvement and develop corrective action plans. \* Promote continuous improvement in services and individual outcomes. \* Ensure Care Base LLC maintains high standards of person-centered care. ## REQUIRED EXPERIENCE & QUALIFICATIONS The ideal candidate should have: \* Experience managing or supervising a \*\*group home, residential program, or HCBS service program\*\*. \* Demonstrated experience working with \*\*HCBS waiver services\*\*. \* Experience supporting individuals with \*\*intellectual and developmental disabilities (I/DD)\*\*. \* Experience supervising direct-support professionals or other healthcare/human-services staff. \* Knowledge of person-centered planning and individual rights. \* Knowledge of service documentation and incident reporting. \* Understanding of residential health and safety requirements. \* Strong organizational and leadership abilities. \* Excellent written and verbal communication skills. \* Ability to work effectively with individuals, families, guardians, case managers, healthcare professionals, and employees. \* Ability to maintain confidentiality and professional boundaries. \* Ability to respond calmly and appropriately to emergencies and challenging situations. \*\*Education:\*\* A bachelor's degree in Human Services, Social Work, Psychology, Healthcare Administration, or a related field is preferred. The Program Manager is expected to maintain regular management hours and be available to address urgent staffing, residential, individual-care, or emergency situations when necessary.
