Hospice Scheduler - Irving, TX
Lead SchedulerThe Lead Scheduler's primary function is to facilitate scheduling of home visits for the field staff each day and assisting with the coordination of patient care. Ensuring geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care. This position will lead the other schedulers and act as a point of contact for the scheduling team.
Essential Job Functions:
Work with intake to push patients to scheduling status as quickly as possible to allow for accurate staffing
Schedule all Starts of Care/Evals and Urgent PRN needs
Monitor the other schedulers for accuracy with plotting scheduling communication notes, scheduler progress throughout the day, monitor referral log for updates on patients and keep statuses updated
Ensures geographic scheduling to reduce drive time and to enhance staff satisfaction and patient care. Schedules all new referrals to the appropriate clinician based upon location of the patient and clinician availability.
Monitor reports for accuracy, correction, notify BCM of concerns for: orphan docs, billing needs, scheduling deviation report, past due visit for consents/NOMNC
Monitor territory status and staffing needs; meet with the BCM and scheduling team daily to determine daily needs
Updates SOC board and goals weekly with office staff
Monitors scheduling chat for urgent clinical needs
Participates in case conference bi-monthly and provides scheduling updates and needs as well as pulls case conference recert list and sends to staff bimonthly
Maintains and creates on-call schedule for clinical staff monthly (sign off to be done by the BCM)
Notify BCM of RN PT CM case numbers, growth patterns, concerns for staffing needs as well as assists BCM as needed with any pilots or special projects
Assist authorization to prevent MV due to auth concerns
Follow-up on outstanding orders, documentation, consents, NOMNCs, as needed daily
Rearrange clinical schedules and reassign visits as needed daily
Coordinate and communicate with PCCs on any new patient need
Assist PCC with DME orders and refer patients to mobile docs when requested
Position Qualifications:
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
