Position Summary:The Licensed Practical Nurse (LPN) – Long-Term Care (LTC) provides direct patient care to residents in a long-term care setting. Responsibilities include administering medications, monitoring patient conditions, and supporting individualized care plans to ensure quality care and patient safety. The LPN works closely with RNs, physicians, and healthcare teams to deliver compassionate, patient-centered care. Key Responsibilities:Provide direct nursing care to residents in a long-term care facilityAdminister medications and treatments as prescribedMonitor patient conditions and report changes to RN/physicianMaintain accurate and timely documentation in EMR systemsAssist with activities of daily living (ADLs) and patient supportPerform wound care and support infection control practicesCollaborate with interdisciplinary teams to ensure continuity of careEnsure compliance with facility policies and healthcare regulationsRequirements:Active, unrestricted LPN LicenseBLS Certification (AHA preferred)Minimum 1 year of LTC experience preferredApplication Process:Please submit your resume.
