PPCCC also provides comprehensive sexuality education programs to help adults and teens make healthy decisions, prevent unintended pregnancies, and avoid sexually transmitted infection.JOB SUMMARY:The Registered Nurse (RN) Case Manager is responsible for initial and on-going assessments of Planned Parenthood California Central Coast Home Health (PPCCCHH) patients assigned to their care, as well as associated administrative responsibilities. In accordance with standards of practice the RN collaborates with the patient and the Home Health Manager to develop, implement, and evaluate the plan of care.Provide visits prenatally to support patients in creating their postpartum planProvide after-pregnancy care to patients who have experienced vaginal delivery, cesarean, miscarriage and/or abortionPerform the initial comprehensive and on-going assessments on each assigned patient’s physical, functional, psychosocial, environmental, and activities of daily living, and initiate preventative and rehabilitative nursing proceduresSupport case management improvement with the successful provision of quality home health care, documentation, and referral, to meet affiliate and community needsParticipate in orientation, competency testing, and in-services of all new personnel, as neededSupport coordination of home health care servicesDevelop and revise the plan of treatment with the patient and physician, as needed. Provide direct care, assessment, and education to patient and family in accordance with the plan of careSecure, document, and coordinate physician orders for needed disciplines and and community services related to the patient’s plan of careAssess and manage patient pain levels by providing education regarding OTC pain medication safety, upper limits, and effective dosage regimensDocument patient service activity in the electronic health record in a timely manner including patient response and progress to goals. Complete documentation at the end of assignments and submit no later than 24 hours after each encounterParticipate in record review and compliance audits as assignedDuring patient/representative interactions identify cultural, spiritual, and verbal/non-verbal patterns indicating a need for individualized service or counselingAssesses each patient’s support system and help them identify alternative options, if neededEvaluate home safety, including infant sleep safety, and recommend solutions for identified concernsPerforms other duties as requestedSUPERVISORY RESPONSIBILITIES:In collaboration with the Home Health Manager provides feedback on non-licensed staff regarding competency and personnel issues. Provide direct supervision and assistance with scheduling, task delegation and coverage planning. Deliver hands-on coaching and on-the-job training to improve performance and offer constructive feedback, document employee progress toward performance goals. Handles direct supervision of ALL clinical staff in the absence of the Home Health Manager.QualificationsPossess a valid California driver's license and a dependable insured automobileAbility to travel within the PPCCCHH designated geographic area (Thousand Oaks to San Luis Obispo)EXPERIENCE AND EDUCATION:Possess one year of professional nursing experience, postpartum experience preferred Graduate of an accredited school of nursingPossess and maintain a current active RN license issued by the California Board of Registered NursingMaintain BLS certificationPossess one year of experience with lactation supportProficiency with PMADs assessmentEnglish-Spanish bilingual skills requiredPlanned Parenthood California Central Coast’s (PPCCC’s) compensation philosophy supports the organization’s mission, vision, and values. Each position has a salary based on market value and the organization’s pay levels. The starting salary for this position is $ 48.24/hour with the opportunity for advancement. As part of our commitment to pay equity, PPCCC does not negotiate salaries.Pay Differential: $1.00/hour bilingual Spanish/English differential pay is offered upon passing the language assessment test.
