Patient Care Pharmacist, Inpatient Pharmacy
Job Responsibilities Review medication orders for appropriateness (dose, route, frequency, indication, interactions, allergies) Verify and dispense physician‐ordered medications in accordance with hospital protocols Perform medication reconciliation upon patient admission, transfer, and discharge Monitor patient therapy outcomes and adjust or recommend changes to optimize treatment Provide drug information to healthcare providers, patients, and caregivers Evaluate and prevent adverse drug events and medication errors Ensure compliance with hospital formulary and therapeutic guidelines Maintain inventory control of medications, including controlled substances Document all activities in the electronic health record (Epic) Ensure compliance with state and federal regulations Precept pharmacy students and residents in clinical and operational settings Position Qualifications Minimum Required: A Bachelor's of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university Licensed as a Pharmacist in New York State or NYS internship permit Preferred: One year experience as a pharmacist Work Environment All work is performed in offices that require working in close quarters with other employees. The pharmacy settings are air conditioned / heated. Adequate protective wear available for handling certain drugs (i.e., chemo) such as a mask, gown and gloves. Gloves and goggles are available for other procedures as necessary.
