Physical Therapist, Outpatient

We are seeking a skilled and motivated mid-level Physical Therapist to join our Outpatient Therapy team in Sierra Vista, AZ. In this role, you will evaluate and treat a diverse patient population, develop individualized care plans, and collaborate with interdisciplinary teams to deliver evidence-based rehabilitation services. This is a full-time position offering a competitive salary and a comprehensive benefits package.Key ResponsibilitiesPerform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.Deliver direct patient care across a range of outpatient diagnoses including musculoskeletal, neurological, and post-surgical conditions.Monitor patient progress and adjust treatment plans accordingly to optimize clinical outcomes.Document all patient interactions, progress notes, evaluations, and discharge summaries accurately and in a timely manner in compliance with facility and regulatory standards.Educate patients and their families on therapeutic exercises, injury prevention, and home exercise programs.Collaborate with physicians, specialists, and other healthcare professionals to coordinate holistic patient care.Maintain compliance with all federal, state, and facility regulations, including HIPAA and safety standards.Required Qualifications (Must-Have)Degree from an accredited Physical Therapy program (DPT or equivalent)Active Physical Therapist license in ArizonaBasic Life Support (BLS) certificationStrong clinical and patient care skillsPreferred Qualifications (Nice-to-Have)Experience in Outpatient, Rehabilitation, or Acute Care settingsPrior experience in an outpatient physical therapy setting.Strong communication and interpersonal skillsAbility to work in a collaborative, team-based environmentBenefitsCompetitive compensation packageHealth, dental, and vision insurancePaid time off and holidays
