Physical Therapist Location: Kaufman, TX Agency: Cynet Health Pay: $2,486 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 5/4/2026 About the Position Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 2 years in Long Term Care setting License: Texas or compact license Certifications: N/A Must-Have: 2 years PT experience in LTC setting; TX or compact license Description: The physical therapist will accurately assess, differentially diagnose, and identify deficits in a patient's ability to move and perform functional activities in their daily lives. They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability. The physical therapist works with the individual to prevent the loss of mobility. Essential Functions and Responsibilities include: Performing patient evaluations by obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data. Establishing an individualized plan of care and selecting treatment techniques that address the specific impairments identified in the evaluation and in accordance with physician orders. Conducting ongoing reassessment of the patient and their response to treatment. Initiating and managing the discharge plan beginning at the first visit and continuing throughout treatment. Supervising and delegating to ancillary staff and students consistent with state laws and department policy. Completing legible, accurate, and clear documentation within the established timeframe identified in department policy and in accordance with accreditation requirements and insurance regulations. Communicating with the patient, family, caregivers, significant others, and members of the healthcare team to promote maximum benefit of care utilizing effective communication strategies. Assessing educational needs of the patient, family, caregiver, or significant other, and providing education to meet those needs. Participating in professional growth and development through participation in educational programs, including in-service meetings, workshops, and by reading current literature concerning the scope of practice. Actively participating in quality improvement activities. Providing an environment conducive to safety for patients, visitors, and staff. Complying with appropriate and approved safety standards. Functioning within the scope of practice as defined by the state's practice act. Performing other related duties as required and directed.
