Registered Nurse
The Registered Nurse plays a key role in delivering patient-centered care for individuals with chronic conditions. This role supports care managers with clinical assessment, triage, and care planning while managing a large, diverse patient population across multiple provider specialties and contracted accounts. The RN will use strong clinical judgment, organization, and adherence to account-specific protocols to ensure continuity of care. This position is also instrumental in helping build and scale Advanta’s RN program as the company grows.
Key Responsibilities:
Conduct clinical assessments, chart reviews, and patient triage
Provide care recommendations, patient education, and clinical guidance
Develop and adjust personalized care plans for chronic condition management
Determine appropriate levels of care (ER, urgent care, or scheduled visit)
Collaborate with care managers, pharmacists, and providers
Ensure accurate documentation and compliance with HIPAA and Medicare CCM guidelines
Support program development, protocols, and clinical training

Credentials:
Education: Bachelor’s degree in Nursing (BSN) from an accredited institution.
Licensure: Active RN license & openness to obtain multi-state licensure
Experience: Minimum of 3 years in acute care, preferably experience with chronic disease and remote work
Billingual (English & Spanish Speaking Only)
Preferred Qualifications:
Experience in managing high patient volumes across multiple specialties, with an ability to follow account-specific protocols efficiently.
Familiarity with chronic disease management, with a focus on primary care conditions, nephrology, neurology, cardiology, pulmonology, etc.
Knowledge of CCM and BHI codes, Medicare regulations, and billing practices for chronic care services.
Pay (Up to $65,000)
