You will provide skilled therapy services in the most meaningful environment possible—the patient's home.Key Responsibilities: Comprehensive Assessment:Evaluate therapeutic, rehabilitative, and functional status; assess muscle strength, mobility, gait, and ROM. Plan of Care:Develop and implement individualized treatment plans in collaboration with physicians and our interdisciplinary team. Home Environment Safety:Evaluate the home for hazards and recommend modifications or assistive devices to ensure patient safety. Education:Instruct patients and caregivers in therapeutic exercises and the use of appliances (orthotics, prosthetics, etc.). Supervision:Oversee Physical Therapy Assistants (PTAs) and Home Health Aides (HHAs) to ensure a cohesive care journey. Clinical Excellence:Maintain accurate, timely documentation (point-of-care) that reflects the high standards of our 5-star rating.The "Archer" Mindset We are looking for therapists who are ambitious, empathetic, and driven by clinical results.Qualifications: Education:Graduate of an accredited Physical Therapy program (DPT preferred). Licensure:Current, unrestricted California Physical Therapist License. Certification:Current BLS/CPR Certification. Experience:Previous home health experience is highly preferred; experience with high-acuity patients is a plus. Mobility:Valid CA Driver’s License with a clean driving record and reliable transportation.
