Pay Rate: $55 - $60 Per hour
Job Overview
We are seeking an experienced Case Manager to join a leading healthcare organization in Carson, CA. The Case Manager will work as a key member of the Interdisciplinary Care Team (IDT), collaborating with physicians, nurses, therapists, social workers, and other healthcare professionals to coordinate comprehensive, participant-centered care. The ideal candidate will have strong experience in care coordination, case management, discharge planning, and working with elderly or medically complex patient populations.
Key Responsibilities
Conduct comprehensive participant assessments in the clinic, home, and virtual settings.
Develop, implement, and maintain individualized care plans in collaboration with the Interdisciplinary Care Team (IDT).
Coordinate care for participants across acute care, emergency departments, skilled nursing facilities, hospice, home health, and outpatient settings.
Perform ongoing care management, participant follow-up, and risk assessments.
Collaborate with Primary Care Providers (PCPs) to manage participant care and clinical outcomes.
Coordinate hospital admissions, discharges, and transitions of care to reduce avoidable hospitalizations.
Review laboratory results, diagnostic testing, referrals, and provider recommendations to ensure timely follow-up.
Educate participants and caregivers regarding disease management, medications, treatment plans, and available community resources.
Coordinate referrals, transportation, durable medical equipment (DME), home health, hospice, and specialty services.
Monitor participant progress and communicate changes in condition to the interdisciplinary team.
Maintain accurate and timely documentation within the Electronic Health Record (EHR).
Advocate for participants while ensuring high-quality, compassionate, value-based care.
Participate in interdisciplinary team meetings, quality improvement initiatives, and care conferences.
Ensure compliance with organizational policies, regulatory standards, and PACE program requirements.
Required Qualifications
Current unrestricted Registered Nurse (RN) License.
Minimum 2 years of nursing experience in a clinical setting caring for frail, elderly, or medically complex patients.
Experience in Case Management, Care Coordination, or Population Health.
Strong clinical assessment and critical thinking skills.
Experience with discharge planning and transitions of care.
Excellent communication, organizational, and problem-solving skills.
Ability to work independently and collaboratively within an interdisciplinary team.
Experience with Electronic Health Records (EHR/EMR).
