Admissions Nurse-RN (Fulltime)

Position Summary:The Admissions Nurse plays a critical role in coordinating and overseeing the admission process for new resides entering the skilled nursing facility. This nurse ensures a smooth, timely, and compassionate transition for residents and their families while verifying clinical appropriateness, gathering accurate health data, and working collaboratively with interdisciplinary teams.Key ResponsibilitiesAdmission CoordinationReceive and review referrals from hospitals, physicians, case managers, and other sources.Conduct clinical pre-admission screenings to access potential residents' needs and ensure appropriate placementCoordinate with the IDT team members including (nursing, therapy, social services, dietary, etc.) to prepare for the resident's arrival.Qualifications Education: Graduate of an accredited nursing program.Licensure/Certification:Current RN license in the state of practiceSkills:Strong clinical assessment and communication skillsAbility to multitask and work in a fast-paced environmentKnowledge of state and federal regulationsExperience with electronic medical records (PCC) systems preferred.
