POSITION TITLE: SERVICE COORDINATORSALARY RANGE: $5,924.00 - $7,206.00 per month in Five Annual StepsNon-Exempt salary (37.5 hours per week)Location: San Jose, Salinas, or Watsonville (Please note the location in your resume)Collective Bargaining Position[x ] Regular [ x] Full-Time [ ] Temporary [ ] Part-timeSend Resume via e-mail to: Email resume to recruiting@sarc.orgJob Summary: While maintaining good public relations and implementing the goals and vision of the regional center, this position is responsible for providing and coordinating services to consumers in accordance with the provisions of the Lanterman Developmental Disabilities Services Act, and policies established by the San Andreas Regional Center Board of Directors.Essential Job FunctionsGathers data and produces initial and/or ongoing assessmentsPresents assessment data to Interdisciplinary Team (IDT) for eligibility determinationCoordinates the Person Centered Planning process with consumers and their familiesDevelops the Individual Program Plan (IPP) or the IFSP for Early Start.Counsels consumers concerning identification and achievement of personal goalsAdvocates with consumers/families to assure provision of needed servicesInitiates and follows up on referrals to existing community resourcesIdentifies areas of unmet needsPerforms tasks which relate to fiscal and accountability requirementsProvides assistance to existing and in the development of new community servicesDevelops and facilitates support groups for consumers and/or their familiesCompletes all required caseload documentationDevelops expertise in accessing generic resourcesAssures that all consumers and families are informed verbally and in writing of their right to appeal every time a decision is made to change, alter, or deny a serviceKnows Title 17 and Title 22 licensing and DDS requirementsKnows all resources that serve our consumers who reside in various districtsIntervenes effectively in crisis situations in order to eventually effect stabilityMonitors residential facilities in compliance with Title 17 regulationsPerforms annual and quarterly visits as required. Monitors day programs to assure quality programmingEvaluates conservatorship requests and submits reports to the appropriate parties.Supervision Exercised: NoneSupervision Received: General supervision and direction by the designated Manager.Education/Experience: A Bachelor’s Degree in social work, rehabilitation counseling or a related field from an accredited college or a Bachelor's Degree in Social Work plus four years of experience. The specific field of study for the Bachelor's Degree and the post Bachelor's years of experience may be waived only by the Executive Director if needed by the agency to meet a special need, e.g. services to specific cultural or monolingual groups.Special Skills and Knowledge: Knowledge of principles, objectives and techniques of social work; individual and group behavior, human growth, development of family relationships; medical-psycho-socio-economic factors affecting individuals and families; techniques of intervention, referral and follow-up to community resources and facilities and various community organizations, and how to intervene constructively on the client's behalf. Ability to relate in a constructive and enabling manner to a wide variety of individuals and organizations; ability to relate in a positive manner with supervisor, co-workers, and others in the agency and community; establish and maintain rapport with clients and their families; prepare and present oral and written reports; maintain case records in a clear, concise, appropriate and timely manner.Other: Use of an automobile, possession of a valid driver's license and automobile liability insurance for the minimum amount prescribed by law, OR ability to provide for independent transportation.MONTHLY STIPEND FOR BILINGUAL SPEAKING!No phone calls please
