OverviewThe Float Advanced Practice Provider provides flexible, high-quality primary care services across multiple clinic locations, filling coverage gaps and supporting continued patient care. Join a place where you can build your career while making a difference and enjoying the balance you need for a full life.ResponsibilitiesConduct comprehensive patient assessments, including history taking and physical examinations across assigned clinic sites.Diagnosis and manage acute, chronic, and preventative health conditions in alignment with evidence based guidelines.Develop and implement patient care plans, including ordering and interpreting diagnostic tests, prescribing medications and initiating referrals as deemed appropriate.Maintain accurate, timely, and thorough documentation in the electronic medical record (Cerner).Collaborates with providers, care coordinators, medical assistants, and other interdisciplinary team members to ensure seamless care delivery.Participate in shared call with primary care providers.QualificationsCompletion of Masters in Nursing or Doctor of Nursing Practice from an accredited programMinimum of 2-3 years of Nurse Practitioner experience within a primary care settingStrong clinic assessment, critical thinking, and decision making skillsExcellent communication and interpersonal skills with patients and care team membersThe above statements are intended to describe the functions and related requirements of persons assigned to this job.
