You will serve as the first point of contact for patients and referral partners, ensuring timely admissions, verifying insurance, coordinating care services, and delivering a seamless transition into home health. You will:Oversee all daily patient intake and referral operationsManage the full intake process, including insurance verification and authorizationEnsure compliance with all state, federal, and payer requirementsCoordinate timely patient admissions in accordance with company policiesBuild and maintain strong relationships with physicians, hospitals, and referral sourcesEnsure smooth patient transitions to home care, including care coordination and educationTrack and report referral data, trends, and performance metricsPromote and maintain a positive reputation in the community as a high-quality providerIdentify opportunities to improve services and patient access to careMaintain knowledge of contracts, payer sources, and community resourcesOur Commitment to YouPerform meaningful work focused on improving patient care and outcomesWork with a collaborative and mission-driven team that celebrates your successOngoing professional development and trainingOur RequirementsCurrent Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of ArizonaHome Health experience a plusExperience in patient intake, admissions, or care coordination preferredExcellent communication, customer service, and relationship-building skillsKnowledge of healthcare regulations and business operationsCurrent CPR certificationThe employer for this position is stated in the job posting.
