Job Description Key ResponsibilitiesYour role is multifaceted, focusing on four core pillars of independence:1. Community Participation Navigation: Assist individuals in safely getting around their local community. Networking: Help build informal support systems and social networks.Skill Building: Teach interpersonal communication skills and help individualsutilize community resources (libraries, parks, community centers).Transportation: Provide instruction and support to help individuals meet theirtransportation needs.2. Health, Safety, & WellnessCoordination: Collaborate with individuals to schedule and attend healthcareappointments.Self-Care: Provide supervision, training, or direct instruction to help individualscomplete personal self-care tasks.Goal Setting: Actively support the person in reaching their personal health andwellness milestones. 3. Household Management Homemaker ServicesMaintenance: Provide guidance and supervision for household care, chores,and general maintenance.Nutrition: Assist with healthy meal planning, grocery shopping, and cooking.Financial Literacy: Offer support with personal needs, purchasing, and budgetingfor the household.Safety: Ensure the individual has the knowledge and skills to maintain a safehome environment.4.
