Yes Does your Facility require physicians to be board certified?: Yes Minimum Board Certification: Eligible What is the license requirement to bid?: Licensed in IN or IMLC Position is 5 days a week but open to 4 days/week; can be somewhat flexible on days of the week to accommodate travel Proposed Schedule includes 1-2 days/wk at hospital clinic and 2 days/wk plus one day/month seeing patients at two satellite clinics (one 15 min from hospital & one 60 min from hospital) Will need IN & IL license
