Plus, earn merit-based raises based on your performance.What We offer:Great culture and team atmosphereComprehensive benefits (medical, dental, vision, life/AD&D, disability)401(k) retirement plan with a generous company matchGenerous time off accrualsPaid holidaysTuition ReimbursementEmployee Referral ProgramMerit IncreasesEmployee Discount ProgramsWhat You'll Do:Receive referrals by phone or in person for designated departmentProvide an explanation of the benefits to patients, families, and community contactsAssist with finding alternative services when the patient is not a candidate for hospiceOversee scheduling of initial nursing assessment and admissionMaintain admission packets and ensure that proper forms are available to admissions staffAssist community outreach representatives with marketing activities to promote relations with referral sourcesAssist with the completion of admission paperwork as neededAdhere to work schedule and maintain good punctuality and attendance recordProvide telephone and clerical support to other staff members as time allowsMaintain accurate clinical records, keeping track of goals and progress of clients'Qualifications:Be eighteen (18) years of age or olderAt least one (1) year experience in homecare, hospice, and/or healthcare-related field preferredAbility to complete necessary documentation appropriately, legibly, and timelyAbility to handle stressful and delicate situationsInterpersonal, verbal, and written communication skills necessary to complete job responsibilitiesValid driver's license and proof of insurance are requiredJ-18808-Ljbffr
