Per Diem Hospice Registered Nurse

Job Overview We are seeking a compassionate and highly skilled Hospice Registered Nurse to provide exceptional end-of-life care to patients in a home or facility setting. The ideal candidate will have extensive experience in palliative and hospice medicine, with a strong background in patient assessment, symptom management, and interdisciplinary collaboration. This role requires a dedicated professional committed to improving quality of life for patients facing serious illnesses, ensuring comfort, dignity, and support for both patients and their families. Candidates should possess comprehensive knowledge of medical documentation, medication administration, and advanced clinical skills including IV infusion, airway management, and sterile techniques. Experience with EMR/EHR systems such as Epic or Cerner is preferred. Responsibilities Conduct thorough patient assessments to evaluate physical, emotional, and psychological needs. Develop and implement individualized care plans in coordination with physicians, social workers, and family members. Administer medications including pain management therapies, IV infusions, injections, and other treatments following strict aseptic techniques. Manage complex symptoms such as acute pain, dyspnea, nausea, and other distressing conditions associated with end-of-life care. Monitor vital signs, perform physical examinations, and document patient progress accurately within electronic health record systems like Epic or Cerner. Educate patients and families on disease processes, medication regimens, advanced directives, and available support services. Provide emotional support and comfort measures to enhance patient quality of life during hospice care. Collaborate with interdisciplinary teams including physicians specializing in hospice & palliative medicine, behavioral health specialists, and case managers. Ensure compliance with HIPAA regulations and infection control protocols to maintain patient confidentiality and safety. Participate in discharge planning when appropriate and coordinate transitions between care settings such as hospitals or long-term care facilities. Skills Proven experience in hospice & palliative medicine with familiarity in managing complex symptomatology such as acute pain management and airway management. Strong clinical skills including medication administration (oral, IV injections), catheter care (including ostomy care), tube feeding management, and sterile processing techniques. Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD-9/10 coding for accurate documentation. Knowledge of laboratory procedures including specimen collection/processing and diagnostic evaluation techniques. Ability to perform comprehensive physical examinations and vital signs assessment with attention to anatomy and physiology knowledge. Experience working with diverse patient populations including geriatrics (memory care), pediatrics (neonatal ventilator), individuals with disabilities or developmental disabilities, dementia or Alzheimer’s care. Skilled in health coaching, discharge planning, infection control practices, and working within NCQA standards for quality assurance. Excellent communication skills for effective teaching hospital experience or outpatient settings; capable of working independently or as part of an interdisciplinary team.
