Licensed Practical Nurse

Job Description Our organization is in search of a committed Licensed Practical Nurse (LPN) to become a part of our Healthcare & Medical Services team at the Health Center in Vermilion, OH. In this position, you will deliver exceptional, resident-focused nursing care within an assisted living, rehabilitation, and memory care setting. Under the guidance of registered nurses and physicians, your responsibilities will include administering medications and treatments, conducting clinical assessments, accurately documenting care, and assisting in the creation and implementation of personalized care plans that enhance residents' physical, emotional, and social well-being.Key ResponsibilitiesMonitor vital signs and identify changes in residents' conditions, reporting them as necessary.Coordinate with interdisciplinary team members to facilitate timely interventions.Assist with wound care, injections, catheter care, and other skilled nursing tasks within your scope of practice.Provide education and emotional support to residents and their families regarding treatment plans and care objectives.Promote safety, infection control, and adherence to all state and federal regulations, as well as facility policies.Work collaboratively with nursing assistants and other care team members, offering direction and feedback to uphold high care standards.Engage with physicians, therapists, social workers, and other healthcare professionals to coordinate services effectively.Participate in care conferences, maintain electronic health records, and contribute to quality improvement initiatives aimed at enhancing resident outcomes and satisfaction.Working EnvironmentAt our Health Center, we cultivate a collaborative, compassionate, and patient-centered atmosphere where your clinical skills and professional judgment are highly regarded.
