Site Physician – Day Treatment Center (DTC)Location: Brooklyn, NY (Rockaway Parkway & Bushwick)Schedule: Monday–Friday | 8:00 AM – 5:00 PM (1-hour lunch)Type: Full-Time | Contract with potential for temp-to-permWe are seeking a Site Physician to provide high-quality, patient-centered care within a Day Treatment Center setting serving adult and geriatric populations. This is a full-time opportunity offering a consistent weekday schedule and the potential to transition into a permanent role.Under the direction of the Regional Medical Director, the Site Physician will oversee the clinical, administrative, and organizational aspects of care delivery, ensuring excellence in patient outcomes and compliance with regulatory standards.What You’ll DoProvide comprehensive primary care to adult and geriatric patientsConduct routine exams, assessments, and follow-up careManage chronic conditions and coordinate specialty care as neededCollaborate with interdisciplinary teams including nursing, social work, and rehab servicesMaintain accurate, timely documentation in compliance with Medicare/Medicaid guidelinesParticipate in care planning and case conferencesMust-Have QualificationsBoard Certified Physician (MD or DO)Active, unrestricted New York medical licenseDEA licenseMedicare & Medicaid provider IDsExperience in Geriatrics, Primary Care, or Internal MedicineStrong clinical judgment and documentation skillsNice to HaveBilingual in Spanish or Haitian CreoleExperience in Day Treatment Center (DTC), PACE, or managed care settingsComfort working with medically complex, underserved populationsAdditional DetailsOrientation conducted in the BronxCoverage across Brooklyn locations (Rockaway Parkway & Bushwick)Stable weekday schedule – no nights or weekendsOpportunity for long-term placement based on performance
