Home Health Registered Nurse (RN)

Job Overview We are seeking a dedicated Registered Nurse (RN) to join our home health care team. The ideal candidate will have experience with Skilled Nursing Facility as well as in a home healthcare and be comfortable with small classroom and lab training. Ideally, we are looking for someone to train basic caregiving, HHA, and CNA. All classes are conducted on site in our training room/lab. Classes are one to two weeks at a time and can be scheduled at intervals to meet your personal schedule. Duties - Facility small classroom and skill performance training in accordance with NMAC and NMDOH requirements. - Instruct in home health administration, adhering to facility/homecare policies and protocols. - Instruct on patient care, medications reminders, and treatment plan documentation in their medical records. - Collaborate with multidisciplinary teams to ensure comprehensive learning. - Monitor patients' vital signs and respond to changes in their condition. - Facilitate/training on MDS (Minimum Data Set) to document patient care and progress. - Implement infection control measures to maintain a safe environment. - Provide education and support to patients and their families. Skills - Experience in Skilled Nurse Facility. - Knowledge of infection control protocols. - Ability to facilitate training and instruct/demonstrate basic HHA skills and equipment use effectively. - Familiarity with MDS and medical records management. - Experience in outpatient home healthcare settings. - Excellent communication and interpersonal skills. - Ability to work collaboratively with other healthcare professionals. - Strong problem-solving and critical thinking skills.
